Within the RN scope of practice as defined by the Oregon State Board of Nursing, applies the nursing process to the care of patients. Teaches patients and significant others. Documents patient care in the medical record. Collaborates with the interdisciplinary team to effectively coordinate overall care of the patient.

  • Able to work in client’s home of various socioeconomic levels with possible exposure to pets, smoke, etc.
  • Successfully integrates OASIS responses into clinical assessments
  • Demonstrates willingness to learn wound and ostomy assessment/management
  • Excellent wound care skills preferred


  • Licensed to practice as a Registered Nurse in the state of Oregon
  • American Heart Association Healthcare Provider Basic Life Support (BLS) or must be obtained by the last day of the month after the 60-day grace period
  • Current driver’s license and automobile insurance
  • Licensed to practice as a Registered Nurse in the state of Oregon
  • BSN preferred
  • American Nurses’ Association certification in Home Health, Gerontology or Case Management preferred
  • Must pass a criminal records check at hire and every three years from the date of the last criminal records check in accordance with ORS443.004


  • Recent (within past 5 years) acute care hospital experience preferred
  • Home health or community nursing experience preferred
